Why proven women’s health innovations still can’t find a home

For more than a decade, femtech’s scale gap has been treated as a funding problem. What if that diagnosis is incomplete?
Despite growing attention, women founders still receive just 2 per cent of global venture funding, and years of advocacy have failed to shift the needle.
This persistence is no longer just a concern; it signals a deeper structural failure.
This is not just a funding gap. It is a system failure.
As Pradeep Kakkitill, founder and CEO of the Health Innovation Exchange (HIEx), argues, the sector continues to operate on a flawed assumption.
The belief that better support to founders alone will unlock scale overlooks the deeper structural constraints that determine whether the innovation is adopted at all.
Barriers That Go Beyond Capital
These insights are not theoretical. Global research reinforces that these challenges are not isolated, highlighting structural, financial and systemic barriers that shape how women-led and under-represented ventures access funding, markets and pathways to scale.
Importantly, these findings are not draw from research alone, but from the lived experiences of women and under-represented founders themselves.
Across HIEx-led interviews and focus group discussions conducted as part of the Reckitt Catalyst Programme, founders repeatedly described the same challenges: fragmented financing, unclear adoption pathways, repeated cycles of proof, and systems that lacked clear routes from validation to procurement and scale.
These experiences suggest that the barriers facing women-led innovation are not simply financial. They are structural.
Many high-potential ventures are not failing because funding is absent.
They are failing because the systems that determine scale, including public procurement, regulation and financing, are not built to move proven solutions beyond pilots into widespread adoption.
This is not a founder problem. It is a system design failure.
Beneath these structural constraints sits a more persistent challenge. Entrenched attitudes shaped by unconscious bias continue to influence decision making.
Across investment and public-sector systems, innovation led by women and underrepresented founders is still frequently perceived as higher risk.
These perceptions shape how opportunities are evaluated, increase the burden of proof placed on founders, and slow decision making. In practice, this results in systematically higher barriers to both funding and adoption.
Systems Unable to Absorb Innovation
Dr. Abas Hassen, lead executive officer for health innovation and quality at Ethiopia’s Ministry of Health, underscores this point.
The primary constraints are not about innovation quality, but about the systems that determine adoption and scale, including procurement, regulation, financing and delivery.
He identifies three persistent challenges: institutional resistance to change, “pilot purgatory” where solutions are repeatedly tested but not integrated into public systems, and a disconnect between what external funders support and what governments can sustain.
Ethiopia’s response reflects a broader shift. Innovation is no longer treated as isolated pilots, but as a structured component of system design.
The country’s system-led innovation model combines regulatory pathways, prioritisation frameworks and structured testing environments to embed innovation directly within the health system.
The implication is clear.
Scaling innovation is not only about accelerating individual ventures alone. It is about strengthening the systems that determine whether innovation is adopted at scale.
The Missing Middle: From Pilot to Procurement
In many low- and middle-income countries, public systems remain the largest market for health and WASH solutions, accounting for the majority of service delivery and procurement.
Yet capital is deployed through models that do not reflect this reality, as scaling depends on public-sector adoption, long procurement cycles and regulatory integration rather than rapid returns.
This creates a misalignment within the financial ecosystem, where capital is structured for faster high returns, while impact depends on long-term system integration.
At its core, the challenge is the absence of clear adoption pathways.
Without structured routes from validation to procurement and system-wide use, even effective solutions struggle to move beyond pilots.
This is the “missing middle”, the gap between early validation and large-scale adoption.
The consequences of this “missing middle” are perhaps best illustrated by the founders trying to navigate it.
Temie Giwa-Tubosun, founder and CEO of LifeBank, describes her decade-old company as an “orphan” within existing financial structures, too commercial for impact investors and too impact driven for venture capital.
Businesses operating within health systems often fall between funding models that were not designed for them.
Thato Schermer, co-founder of Zoie Health, describes a similar challenge.
Even companies with strong revenue and clear demand struggle to secure funding at the right stage, as they are assessed through frameworks that do not reflect the healthcare markets.
Across interviews and focus group discussions, these patterns were consistent.
Founders described fragmented financing, unclear adoption pathways, and repeated cycles of proof, where they are asked to keep proving their solutions without a clear route to scale.
These are not isolated challenges. They reflect how innovation is funded, evaluated and integrated across the system.
The barrier to scale is not a lack of viable solutions. It is about the systems and models that are not designed to support them.
Reducing Risk Through System Design
From an HIEx perspective, a different approach is emerging, one that focuses not on fixing founders, but on designing how systems manage risk and adopt innovation.
Rather than avoiding risk, Ethiopia is working to manage it through structured processes.
The system is “risk-aware, not risk-averse.” It uses innovation sandboxes, structured testing environments within public systems that allow new solutions to be evaluated under controlled conditions.
These mechanisms, generate decision-grade evidence while limiting system-wide exposure, creating clearer pathways from validation to adoption.
When innovations are tested within public systems, they gain institutional legitimacy. This reduces perceived risk for both governments and investors and enables more confident decision making.
From Fragmentation to Coordination
Within this context, initiatives such as Reckitt Catalyst, a multi-partner platform supporting women-led health and WASH innovation to scale, play a critical bridging role.
By connecting entrepreneurs with governments, investors and technical partners, and aligning solutions with national priorities, the programme helps to create clearer pathways from pilot to procurement and scale.
But alignment alone is not enough.
As Pradeep Kakkattil notes, the climate movement offers a useful parallel. Climate progress was not driven by evidence alone. It accelerated when investors, governments, and institutions began treating inaction as the greater risk.
Sustained pressure exposed the cost of doing nothing, redefined how risk was assessed and ultimately reshaped capital allocation and policy decisions.
Women’s health and WASH innovation is now at a similar inflection point.
Despite years of evidence and advocacy, outcomes such as women receiving a fraction of global funding persist.
This is not due to a lack of solutions. It is because the systems governing investment, adoption and scale have not been sufficiently challenged.
What is required is not incremental progress.
It is a shift in what the system tolerates – how risk is defined, how capital is allocated, and how accountability is enforced.
A System at an Inflection Point
The implications are clear.
Investors must move beyond rigid funding models and deploy capital aligned to how health systems scale. Governments must build clearer pathways for testing, procurement and adoption.
Ecosystem actors must shift from supporting individual ventures, to enabling system-level integration.
The persistent funding gap is not a result of slow progress; it reflects a system operating exactly as designed. Incremental change will not shift outcomes.
What is required is a fundamental reset of how femtech is financed and scaled: from passive investment to active market-shaping, where capital, policy, and procurement work together to create real pathways to adoption.
Until that shift happens, the sector will continue to produce innovation that the market is not structured to absorb.

Spain football federation partnership to offer players egg-freezing and fertility treatment

Spain’s football federation will offer national team players egg-freezing and fertility treatment through a clinic partnership running until 2029.
The agreement with the Tambre clinic in Madrid will give Spain’s World Cup winners more options to pursue motherhood when they feel the time is right.
The Royal Spanish Football Federation (RFEF) said the agreement covers all national team players, including the futsal team, as well as Primera Division referees and assistants and all other federation employees.
It is the latest policy introduced by the RFEF as it seeks to lead the way in work-life balance and maternity protection.
The federation has also introduced measures allowing players to travel with their children during major tournaments. The scheme was extended to the men’s team during their successful World Cup campaign in North America this past summer.
RFEF president Rafael Louzán said: “If Spain and its national teams are the best in the world on the pitch, they must also be the best in matters such as work-life balance.
“This is a new, comprehensive and pioneering benefit that extends to all federation employees. Today is a great day for the entire Spanish football family.”
The agreement followed a direct request from players after discussions with national team captains, including London City Lionesses midfielder Alexia Putellas.
Putellas told Spanish outlet El País: “As a group of captains, we had a concern — because experiencing a World Cup is vital for a player — regarding the difficulty of reconciling that with the desire to become a mother.”
“It’s a desire I didn’t originally have but developed some time ago.
“Thanks to the incredible work done by the federation, an agreement has been reached with a clinic allowing us to freeze our eggs while we are still active players, for as long as we wish.
“For me, this is a huge step forward; the federation understands that you are serving your country in a way that runs counter to your biological clock — that you can’t simply pause your career to become a mother because you need your body to do your job.”
The announcement follows Chelsea becoming the first Women’s Super League club to offer similar fertility treatment options to its players.
The move has also drawn criticism.
Spanish triple jumper Ana Peleteiro-Compaoré Brión said on Instagram: “The problem obviously isn’t freezing eggs.
“The problem is: why does the institution these women work for have an interest in financing them to delay their motherhood?
“And here there’s a problem that for me changes absolutely everything.
“Is this measure intended to make it easier for women to be mothers or is it intended to make it easier for them not to be while they’re playing?”
Spain are the current world champions in both the women’s and men’s games.
The women will attempt to defend their title at the 2027 World Cup in Brazil next summer.

High street bakery chain Gail’s reveals menopause plan

Gail’s has unveiled a menopause action plan offering new workplace support to thousands of staff ahead of legal reforms due in 2027.
The high street bakery chain will offer new benefits including 24/7 digital GP access and a new employee assistance programme, according to The Times.
It will also explore new online training for staff, including specific training for management.
Gail’s people director Miranda Burgum told The Times: “All we’re trying to do is talk freely and for it not to be a forbidden subject. It’s helping our managers and teams understand that this isn’t a taboo.”
“We’re going to develop the education with our managers, so it will be threaded through all our policies
“It will look at induction training, it will look at the types of people we need to make sure that we make reasonable adjustments for.
“And if somebody needs some sort of risk assessment, we’re going to be looking at [that].”
The move comes ahead of reforms due to be introduced in spring 2027 under the Employment Rights Act.
UK employers with 250 or more employees will be legally required to publish and update official menopause action plans.
The plans are intended to support employers to take effective action to improve workplace gender equality and support employees experiencing menopause.
Fertility
Paracetamol use may impact future fertility, studies suggest

Paracetamol use in pregnancy was not linked to autism or ADHD, while separate research found reproductive differences in girls exposed before birth.
One study analysed health records from more than 120,000 children and found no increased risk of autism following prenatal paracetamol exposure.
A separate analysis of nearly 100,000 children also found no increased risk of ADHD among those born to mothers who used the painkiller during pregnancy.
Researchers from the Hong Kong Hospital Authority examined electronic health records covering pregnancies between January 2001 and December 2023.
The autism analysis included 124,333 children, who were nine years old on average and split almost evenly between males and females. There were 3,445 autism diagnoses, representing 2.8 per cent of the group.
The ADHD analysis involved 97,285 children, who were seven years old on average and also split evenly between males and females. There were 5,168 ADHD diagnoses, representing 5.3 per cent.
Women prescribed paracetamol during pregnancy were more likely to be older and have pre-existing conditions including psychiatric disorders, as well as reasons for taking the drug such as infection, fever or chronic pain.
No association was found between prenatal paracetamol exposure and either autism or ADHD.
The findings did not differ according to the trimester in which paracetamol was taken or whether use was intermittent or daily. Advanced maternal age, defined as pregnancy in women over 35, did not alter the findings.
The researchers wrote: “Paracetamol remains a safe and essential analgesic [pain reliever] and antipyretic [fever reducer] during pregnancy, whereas alternatives, such as NSAIDs and opioids carry well-documented risks.
“Unwarranted reluctance to use paracetamol could lead to undertreatment of pain and fever, or the use of more harmful alternatives, both posing risks to the pregnancy and developing fetus.”
The authors said women should assess paracetamol use with guidance from their doctor.
A separate study involving 685 pregnant women without pre-existing conditions and 302 infant daughters found associations between prenatal paracetamol exposure and differences in reproductive organs and hormone levels.
Researchers from Copenhagen University Hospital enrolled the women during their first trimester and assessed them during the first trimester, third trimester and again when their babies were three months old.
At around three months, infants experience a temporary rise in reproductive hormones sometimes called mini-puberty.
Pregnant participants completed questionnaires every two weeks about their use of pain medicines including paracetamol. Infant girls underwent ultrasound scans of their reproductive organs and blood tests to measure hormone levels.
Researchers also examined a separate group of 1,210 girls followed from infancy to adolescence whose mothers reported paracetamol use during the third trimester.
Three-month-old girls exposed to paracetamol before birth had an average 40 per cent smaller ovarian volume, 13 per cent smaller uterine volume and 23 per cent fewer ovarian follicles.
Girls exposed during the first trimester also had lower levels of Anti-Müllerian hormone, a marker of ovarian function.
Among the older girls, those exposed before birth were more likely to have smaller uteruses at puberty and smaller ovaries during their teenage years.
Dr Margit Bistrup Fischer, lead study author and postdoctoral researcher in the Department of Growth and Reproduction at Rigshospitalet hospital in Denmark, said: “Animal studies have demonstrated that impaired formation of ovarian follicles can lead to reduced fertility and earlier reproductive aging.
“Whether the differences observed in our study have implications for fertility and age at menopause in humans remains unknown and will require long-term follow-up of the girls in our cohort.”
She cautioned that women who had used paracetamol during pregnancy “should not be alarmed by our findings”, as the study found associations rather than direct causation and outcomes for individual women and children are unclear.
“Importantly, our study does not evaluate whether [acetaminophen] causes reproductive problems, nor does it provide evidence that prenatal exposure affects future fertility or age at menopause,” she said.
“Although we observed similar associations in an independent cohort, long-term follow-up is needed to determine whether these early-life differences have any clinical significance later in life.”
